Just Launched! CCD Integration

CSS is excited to announce that we have just launched our Continuity of Care Document (CCD) Integration.

CCD Integration moves CSS and MTMPathTM significantly along the interoperability path.  With the new CCD Integration functionality, medication and care management clinicians using the MTMPathTM platform can now integrate with any physician and hospital electronic record system to consume CCD documents and effortlessly build a patient profile for medication management review.  CCD integration:

  • Allows for robust therapy gap identification
  • Reduces the administrative burden of data entry and maximizes valuable pharmacist time
  • Allows clinic based pharmacists to significantly scale their medication management efforts over large populations of patients

To launch its CCD Integration capability CSS has partnered with the Rio Grande Valley HIE.  RGVHIE aggregates CCDs from its partner clinic organizations.  Clinicians located at partner facilities can request a medication review by a clinic pharmacist using the MTMPathTM platform by submitting a CDD transmission request.  The CCD request routes a CCD from RGVHIE to MTMPathTM where it is consumed and a patient profile is prepared with demographic, medication, diagnosis and lab data for medication management review.

CCD Integration reduces the data entry burden for medication management clinicians, allowing clinicians to focus on identifying and remediating care and therapy gaps.

The CCD Integration can be deployed in any environment where a physician or clinic EHR can submit a CCD file.  CCD Integration makes MTMPathTM the premier platform for conducting medication management in physician office, integrated delivery system and accountable care environments.

CCD Basics

The CCD is built using HL7 Clinical Document Architecture (CDA) elements and contains data that is defined by the ASTM Continuity of Care Record (CCR).  CCDs are used to share summary information about a patient between clinicians using EHR, medication management and care management technology.  The CCD uses a detailed set of templates to describe the data elements which can be transmitted.  Each template defines how to communicate a clinical data type.  CCD templates include:

  • Header
  • Allergies
  • Problems
  • Procedures
  • Family history
  • Social history
  • Payers
  • Advance directives
  • Medications
  • Immunizations
  • Medical equipment
  • Vital signs
  • Functional stats
  • Results
  • Encounters
  • Plan of care

As always I am happy to discuss your medication therapy management program and CCD integration.  CSS supports ACOs, integrated care delivery systems and physician groups to design, implement and manage medication management programs.  Feel free to reach out by phone or e-mail.
